Table-of-contents (index) file

A file that lists a reporting entity's plans and links each one to the in-network and allowed-amount files that apply to it.

Large carriers rarely publish one file per plan. Instead they publish a table-of-contents file, often called an index file, that maps plans to shared rate files. Many plans point at the same in-network file because they use the same provider network.

The index is the starting point for any MRF crawl: it tells you which files exist, which plans they cover, and where to download them.
